Transcript Services

Unofficial Transcripts:

Unofficial transcripts are free of charge, and can be requested in person, by mail, fax, or viewed online through www.maricopa.edu/students located in the Student Center under "Academic Records."

Official Transcripts:

Official transcripts are $5.00 for each copy requested. Official transcripts are embossed with the College Seal and printed on official transcript paper. Transcripts provided to Maricopa Community Colleges are free and require the transcript request form for processing.

Home School High School Transcript Guidelines

For authentication purposes, the PVCC Honors Office will review your high school transcript as instructed by the Maricopa Community College District.

The home school high school transcript must clearly display the following information:

1. Student’s name (first, middle, last) and Date of Birth
2. For each course completed, include the following:
   1. Specific course title
   2. Units of credit for each
   3. Time frame or semester in which completed
   4. Grade received
3. Description of grading system used such as a grade scale or grade key
4. Graduation date (month/day/year) indicating completion of secondary school education
5. The home school administrator is the person who organized, taught, and evaluated course work. If the home school administrator is not the parent named in item 7 below, include the name of the home school administrator
6. The following statement, or one very similar, is to appear above the parent signature line described in item 7 below:
   
   I, the undersigned, do hereby solemnly attest that this student has completed a secondary school education in a home school setting on the date indicated and under the laws governing the state and county in which the home schooling occurred.
    
7. Name and address of parent or legal guardian with a space for the signature. The transcript is to be signed in the presence of the Notary Public (see item 8 below)
8. The transcripts must be notarized; to include:
   1. Notary Public Signature
   2. Printed Name
   3. Date
   4. State / County
   5. Date Commission Expires

The transcript containing the original notarized seal must be mailed by the preparer directly to:

To request a Transcript Evaluation for course work taken outside of the Maricopa Community College District, please verify that PVCC Admissions and Records office has received your official transcript. Once the transcript has been evaluated, those credits will be posted in the Student Center under the transfer credit report.  Please allow up to 12 weeks for an official review. During peak season, this time may vary.

Paradise Valley Community College does not evaluate any foreign transcripts. It is not required to have your foreign transcript evaluated, however, you may want to have it evaluated if you wish to do either of the following:

• To prove that you have already taken prerequisite courses.
• To use your credits from a foreign institution towards your degree from PVCC.
  *There is no guarantee the foreign classes will transfer in to our school.

If you choose to have your transcript officially evaluated, please review the List of International Credential Evaluation Agencies used by many colleges and universities across Arizona. If you plan to transfer to a university, confirm with the university on transferability.

After transcripts have been evaluated by one of the International Credential Evaluation Agencies, please have the evaluation sent directly to:
