Incident Reporting Form

 

The Office of the Vice President of Student Affairs and Office of the Dean of Students are responsible for investigating violations to the Student Code of Conduct as well as claims of Sexual Harassment or Discrimination involving students.

For instances of student misconduct, sexual harassment, or discrimination involving students, please complete the Incident Reporting Form at the link below. Remember to provide as many details as possible and avoid diagnosing behavior or using inflammatory language to describe the situation.

Please click on the link below and your referral will be routed to the Office of the Vice President of Student Affairs and assigned to the appropriate adjudicator or investigator.

Instances of student misconduct will be assigned to the Dean of Students or designee, and instances of Sexual Harassment and Discrimination will be routed to the VPSA and Title IX Coordinator and assigned to a trained Title IX Investigator.
