How to report Coronavirus (Covid19)

Employees

 

When to report? IMMEDIATELY for any of the following conditions

  • As soon as COVID like symptoms appear (no matter how minor)
  • As soon as you are notified that you had a potential exposure.
  • If you are asymptomatic but decide to test, report as soon as you get a positive diagnosis.

DO NOT:

  • Wait until you test for COVID-19 to report.
  • Wait until you receive your test results to report.
  • Do not automatically return to campus, if a negative test result.

Individuals with COVID-like Symptoms or tested positive for COVID-19: (Regardless of Vaccine Status)

Isolate yourself for a minimum of 5 days and monitor your symptoms. You are able to return to in-person classes/work when all 4 of the following CDC criteria have been met:

  1. At least 5 days since symptoms first appeared and
  2. At least 24 hours with no fever without the use of fever-reducing medication and
  3. Other symptoms of COVID-19 are improving or have completely resolved (and no new symptoms appear) and
  4. Continue to wear a mask/face covering around others for 5 additional days after isolation is discontinued.

After you have filled out the report: 

Students – email both your instructors, and covid19-reports@paradisevalley.edu
Employees – email both your Supervisor, and covid19-reports@paradisevalley.edu 

 

Potential Exposure to a COVID-19 positive individual:

If you are asymptomatic and any of the following scenarios apply to your situation:

Completed the two-dose vaccine series for either Pfizer or Moderna within 6 months of your potential exposure or

You do not need to quarantine, but you must wear a mask around others for 10 days.**

Completed the primary series of the J&J vaccine within the last 2 months from your potential exposure or

You have had any of the booster shots or

You have recovered from COVID-19 within 3 months of your potential exposure.

If you are asymptomatic and any of the following scenarios apply to your situation:

Completed the primary series of Pfizer or Moderna vaccine over 6 months ago and are not boosted or

Stay home for 7 days from the date of your last exposure. After that continue to wear a mask around others for 5 additional days.**

Completed the primary series of the J&J vaccine over 2 months ago and are not boosted or

Are unvaccinated or

Have not recovered from COVID-19 within 3 months of your potential exposure.

Vaccinated or not you will still fill out the following form:

After you have filled out the report:

**If you develop COVID-19 like symptoms or test positive during your quarantine, regardless of your vaccine status, you must isolate and immediately complete the online form Report Symptoms or Confirmed Case.

We are confident in our cleaning procedures but letting us know if you’ve been on campus while sick will allow us to apply extra steps to those spaces which have been exposed.

I need office, etc., supplies sent to my home.  What’s the process?

  • Items can be ordered by procard or by purchase order.
  • All orders must be sent to the college address and picked up at the college Receiving department. Do not ship items to a home address.
  • Receiving hours are Monday-Friday: 9:00 am—3:00 pm , closed for lunch 12:00 pm—1:00 pm
  • If working on campus, you may pick up items from Receiving or you can contact Lance Miller, 602-787-7372 to have packages delivered to your campus work area.

What About Mail?

  • Mail will be held at the Copy Center in KSC-1506 for individuals to pick up.  Mail will not be forwarded to anyone.  You can call ahead to let them know you will be picking up your mail:  602-787-7770
  • The PVCC Copy Center is open and can process copy and print jobs submitted by the online request forms at https://www.paradisevalley.edu/employees/copy-center.
  • The copy center will not mail print and copy center requests. They must be picked up.
  • The copy center hours are
    • Mondays  - Thursdays: 7:00 am—6:00 pm
    • Fridays: 7:00 am—4:00 pm

Return to Workplace Training

Employees are required to complete this training prior to returning to campus. All employees were automatically enrolled in the Return to Workplace Training plan in the Employee Learn Center (ELC). Once the training is complete it will display on your ELC transcript and trigger an email notifying your supervisor that you may be on campus per MCCCD guidelines.
